Swift’s Active B12 blood test costs £55. You can have it at clinics across Essex, plus Bromley and Sunbury, or at home. Active B12, also called holotranscobalamin, measures the part of your vitamin B12 that your cells can actually use. It can be especially helpful when a total B12 result is borderline.
Why B12 matters
Vitamin B12 helps your body make healthy red blood cells and keeps your nerves working properly. Low levels can develop slowly over months or years. Symptoms are often vague at first, which is why many people don’t connect them with B12.
Common signs of low B12 include:
- tiredness and lack of energy;
- pins and needles, tingling or numbness;
- a sore, red tongue or mouth ulcers;
- problems with memory or concentration;
- feeling breathless or looking pale.
Neurological symptoms, such as numbness, balance problems or memory changes, need prompt attention from your GP. Testing shouldn’t delay that.
Who is most likely to benefit from testing
- Vegans and vegetarians, because B12 is found mainly in animal foods.
- People taking metformin or acid-reducing medicines such as proton pump inhibitors. Don’t stop these without advice.
- People who have had stomach or bowel surgery, or who have coeliac disease or Crohn’s disease.
- Older adults, who absorb B12 less efficiently.
- Anyone with the symptoms listed above.
Active B12, total B12 or a panel?
| Option | Price | Choose it if |
| Active B12 | £55 | You want the more specific marker, or a previous total B12 result was borderline |
| Vitamin D, B12 & Folate | £115 | You want the common nutritional causes of tiredness checked together |
| Advanced Iron | £120 | You suspect anaemia; it includes B12, folate and a full blood count |
The Vitamin D, B12 & Folate blood test covers three common causes of low energy in one appointment. The Advanced Iron blood test is the better choice if anaemia is a concern.
Why folate matters alongside B12
Folate and B12 work closely together, and a lack of either can cause a similar type of anaemia. Taking folic acid on its own can hide B12 deficiency anaemia while nerve damage continues. That’s why the two are often tested together. If you only need folate checked, Swift also offers a folate blood test.
Metformin and B12
The MHRA’s June 2022 Drug Safety Update says low vitamin B12 is now considered a common side effect of metformin, especially at higher doses or with longer use. If you take metformin and have symptoms of low B12, keep taking your medication and ask your GP about testing.
Preparing for your test
- If you take B12 supplements or have B12 injections, your result may reflect your recent dose. Tell the clinician when you last took one.
- Don’t stop prescribed injections or tablets for the test.
- Follow any fasting instructions given when you book.
Clinic or home collection

How booking works

- Receive your results and share them with your GP.
Foods high in B12
B12 is found naturally in animal foods. Good sources include:
- meat, especially liver;
- fish and shellfish;
- eggs;
- milk, cheese and yoghurt.
If you eat few or no animal products, fortified foods such as some plant milks, breakfast cereals and yeast extract can help. Vegans usually need fortified foods or a supplement to get enough B12.
Normal B12 levels and reading your result
Normal B12 levels are the same for women and men. Your report shows the laboratory’s reference range for active B12, and ranges vary between laboratories, so always compare your result with the range on your own report. A low result suggests your cells may not be getting enough B12. A borderline result may need repeating or further tests. Your GP can decide whether treatment is needed and, if so, whether tablets or injections are best.
Frequently asked questions
What is an active B12 test?
It measures holotranscobalamin, the form of vitamin B12 available to your cells.
Is active B12 better than total B12?
It can help when total B12 results are borderline. Your clinician interprets either result alongside your symptoms.
How much is a private B12 test?
£55 for Active B12 at Swift.
Should I stop B12 supplements before testing?
Ask your clinician. Don’t stop prescribed injections.
Can metformin lower B12?
Yes. Low B12 is a recognised common side effect of metformin. Keep taking your medication and discuss testing with your GP.
