Key Test Components

FSH (Follicle Stimulating Hormone)
  • FSH is produced by the pituitary gland and plays a key role in stimulating the growth and maturation of ovarian follicles, which produce eggs and estrogen. Levels fluctuate throughout the menstrual cycle and rise during perimenopause or menopause as ovarian function declines. Elevated FSH can indicate reduced ovarian reserve, while low levels may suggest pituitary or hypothalamic issues.
  • LH is secreted by the pituitary gland and works in conjunction with FSH to regulate the menstrual cycle. It triggers ovulation by causing the release of a mature egg from the ovary and supports the production of progesterone. High LH levels are associated with conditions like PCOS, while low levels may point to pituitary dysfunction.
  • Estradiol is the primary form of estrogen produced by the ovaries, essential for regulating the menstrual cycle, maintaining bone density, supporting cardiovascular health, and influencing mood and skin. Levels peak during the follicular phase and drop in menopause. Abnormal levels can contribute to irregular periods, infertility, or symptoms like hot flashes.
  • Progesterone is mainly produced by the corpus luteum after ovulation and prepares the uterine lining for pregnancy while supporting early gestation. It rises in the luteal phase of the cycle and falls if pregnancy doesn't occur, triggering menstruation. Low levels may indicate anovulation or luteal phase defects, affecting fertility.
  • Total testosterone measures the overall amount of this androgen hormone in the blood, produced by the ovaries and adrenal glands in women. It influences libido, muscle mass, energy levels, and mood. Elevated total testosterone can be a marker for PCOS or adrenal disorders, leading to symptoms like hirsutism or acne.
  • Free testosterone represents the unbound, biologically active fraction of testosterone not attached to proteins like SHBG or albumin. It provides a more accurate assessment of androgen activity than total testosterone alone. High free testosterone levels are linked to hyperandrogenism, contributing to irregular cycles or masculinizing effects.
  • SHBG is a protein produced by the liver that binds to sex hormones like testosterone and estradiol, regulating their availability to tissues. High SHBG can reduce free hormone levels, while low SHBG increases them. It's influenced by factors like estrogen, thyroid function, and insulin, and helps interpret androgen-related conditions.
  • Prolactin is produced by the pituitary gland and is primarily involved in milk production during lactation, but it also affects reproductive function by suppressing ovulation when elevated. High prolactin (hyperprolactinemia) can cause irregular periods, infertility, or galactorrhea, often due to pituitary adenomas, medications, or stress.

Note:

For menstruating women, collection is recommended on day 2-4 of the menstrual cycle (day 1 is the first day of bleeding) for baseline levels. Post-menopausal or non-menstruating women can collect anytime. Morning collection (7-10 a.m.) is preferred as some hormone levels peak then. No fasting is required.
